Case Management

• Collaborative care coordination with community nutritional services (meal delivery), Home Health Care, Personal Care Aide agency, and scheduling appointments. 

• Develop a Plan of Care

• Individualized disease management coordination with DME, company to work with Primary Care Physician to obtain orders for equipment and devices to aide with ADL's and/or incontinence supplies

Personal Care Aides (PCA)

• Aide with routine activities of daily living

• Personal care - bathing and oral care

• Incontinent and hygiene care

• Dressing & undressing

• Meal preparation

• Housekeeping - clean soiled linens, take trash out, clean bedside commode/bedpan/urinal, bathing area

• Clearing paths of hazards in general living areas for safety

• Laundry for cleanliness (per care plan)

• Errands services - pick up groceries, over the counter medications, postal mail, laundry (per care plan)

• Respite Care for caregiver
